Some patients tolerate azithromycin without difficulty. Others develop gastrointestinal disturbances, hepatic enzyme elevations, or, more rarely, signs of a serious allergic or hypersensitivity reaction. When adverse effects are mild, a physician may advise continuing the course with supportive care. When they are severe—particularly when liver function tests return with significantly abnormal values, or when a patient develops symptoms consistent with a hypersensitivity reaction such as drug rash with eosinophilia and systemic symptoms (DRESS)—discontinuation becomes not just appropriate but urgent.

It is worth noting that azithromycin-associated hepatotoxicity, while uncommon, has been documented in the medical literature. Patients who develop jaundice, severe abdominal pain, or marked fatigue during a course of azithromycin should contact their physician immediately rather than waiting to see whether symptoms resolve on their own.

The Question of Treatment Efficacy at Day Two or Three

A more nuanced clinical scenario arises when a patient's condition deteriorates despite being on azithromycin. If a physician prescribed the drug for community-acquired pneumonia or an exacerbation of chronic obstructive pulmonary disease, and the patient's clinical status worsens rather than stabilizes within the expected timeframe, this trajectory raises questions about whether the antibiotic is working. In some cases, the deterioration signals that the infection is caused by a resistant organism, a pathogen outside azithromycin's coverage spectrum, or a non-bacterial process entirely—such as a viral illness that was misidentified as bacterial at the outset.

In these circumstances, the physician may stop azithromycin, pursue additional diagnostic workup, and initiate a broader-spectrum or alternative agent while awaiting results.

What Patients Should Do When Instructed to Stop

Receiving a mid-course discontinuation instruction can feel counterintuitive, particularly given the longstanding public health messaging around completing antibiotic courses. Patients who are told to stop should resist the impulse to continue taking the remaining pills independently, reasoning that finishing the prescription is the safer choice. It is not. Continuing a medication that your physician has identified as inappropriate—whether due to resistance, interaction, or adverse effect—can cause harm and does not contribute to antibiotic stewardship in any meaningful way.

The remaining pills should be disposed of properly, using a local drug take-back program or following FDA guidelines for safe household disposal. They should not be saved for future use or shared with others.
